This document contains a health care power of attorney where you can name a healthcare agent and a couple of alternate agents who would have the authority to make healthcare decisions for you if you are unable to make them for yourself.
It also contains the Living Will where you can state ahead of time your treatment preferences should you be in a terminal condition or in a state of permanent unconsciousness. Last, it also gives you the opportunity to state your preference for your guardian should it ever be determined that you need a guardian named for you.
